To ensure coordination between the NYC Heath and Hospitals (H+H) Correctional Health Services (CHS) Division and the NYC Department of Homeless Services (DHS) in expeditiously placing Brad H Class Members into a Program Shelter, including a Mental Health shelter, as appropriate, upon their release from the Rikers Island Correctional Facility, per the stipulations laid out in the Brad H. et al, v. the City of New York Stipulation of Settlement.
