The purpose of this policy bulletin is to inform Workers at Job Centers that the list containing all forms required for the Public Assistance Application Kit Forms (M-90c), which was first published on September 1, 2006 as part of the Job Center Operations Manual, has been revised to reflect the following: The Child/Teen Health Program Fact Sheet (W-127K) is obsolete and has been replaced with the Child/Teen Health Program Fact Sheet (MAP-1096), & the Child Care Fact Sheet (W-574EE) is obsolete and has been replaced with the Child Care Fact Sheet (ACS W-574EE).
