#09-67-OPE: Revision To The NCA FS Application/Recertification Kit Forms
The purpose of this policy bulletin is to inform Non Cash Assistance Food Stamp (NCA FS) Center staff that the NCA FS Application/Recertification Kit Forms (M-90e) has been revised to include the New York State (NYS) How To Use Your Benefit Card To Get Food Stamp and/or Cash Benefits (PUB-4596). NCA FS Center Managers must ensure that all previous versions of form M-90e are removed from circulation and recycled.
