The purpose of this memorandum is to distribute the attached 1995 rates and rate codes to be used in the prior authorization of the Medicaid (MA) funded home care services component of the Assisted Living Program (ALP). All reimbursement for 1995 will be retroactive and automatically generated to the ALP.
