The purpose of this INF is to introduce the revised Statewide 5/05 versions of LDSS 4826:“Food Stamp Benefits Application/Recertification ” and LDSS-4826A :“How To Complete The Food Stamp Benefits Application/Recertification ” and to inform local districts that several “other than English ” versions of the forms have been added.
