The purpose of this release is to introduce revisions to the worker-completed eligibility determination form DSS-3570: "Certification Guide"
(copy attached). The Certification Guide was restructured and the questions
were reordered to reflect the changes made in 1989 to the "Application for
Public Assistance, Medical Assistance, Food Stamps, Services". A detailed
listing of the revisions is attached.
