The purpose of this release is to introduce revisions to the
applicant/recipient-completed eligibility forms and their accompanying
publications (copies attached). The upstate and New York City Applications were expanded from four pages to six pages so that when Page Five is folded back, the information for each person is carried all the way across the top of Pages Two, Three and Five. All "How To Complete" publications were enlarged to 8½" x 11".
