The purpose of this release is to inform you that the final versions of the
judicial and extra-judicial surrender forms for Section 383-c of the Social
Services Law (SSL) are available. Both the English and Spanish versions of
the forms are available. The forms reflect the change made in statute,
whereby the surrendering parent(s) may be contacted anytime after the child
reaches the age of eighteen years eighteen years if both parties agree, instead of after
the child reaches the age of twenty one.
