This directive transmits a new home care assessment instrument and the
instructions for completing the instrument. The directive also includes
the implementation schedule, the procedures for reassessing certain
personal care services recipients, and the letters for notifying
recipients of the reassessment requirements and the outcome of the
reassessment. A separate directive will discuss the use of the new home
care assessment instrument by certified home health agencies.
These policies and procedural changes are required by Social Services
Law 367-o (Chapter 41 of the Laws of 1992).
