GIS 02TA/WMS004: Periodic Reporting—New Report Form

GIS 02TA/WMS004 Explains that upstate recipients of public assistance and food stamps who are subject to periodic reporting will be sent a newly-formatted “PERIODIC REPORT”, LDSS-4310. This new form replaces the former LDSS-4310, “QUARTERLY REPORT”.
The GIS states that no changes have been made to selection criteria and, as previously, recipients must return the completed form by the 10th of the month following the mailing of the form.
