10 OLTC/009: 2010 Consumer Directed Personal Assistance Program (CDPAP) Implementation Plan

This GIS message will serve as a reminder that pursuant to 09 OLTC/LCM-1 Chapter 58 of the Laws of 2009 Amending Social Services Law (SSL) 365-f Consumer Directed Personal Assistance Program (CDPAP) each social service district must annually submit a CDPAP Implementation Plan to the Department. The plan is due no later than November 30, 2010. An electronic copy of the required form is available at
http://health.state.nyenet/docs/cdpap/cdpapplan.pdf. You should submit your completed CDPAP Implementation Plan electronically at
