10 OLTC/008: Assisted Living Program (ALP) Model Contract (for use outside of NYC)
The purpose of this GIS is to advise local social services districts (LSSD) commissioners and long term care staff of the inclusion of the Assisted Living Program (ALP) Model Contract (for use outside of NYC) and the related attachments on the Department of Health's intranet website.

The revised ALP Model Contract and related documents can be obtained through the Intranet at the DOH OHIP, Library of Official Documents link at: http://health.state.nyenet/revlibrary2.htm. The Office of Long Term Care ALP Model Contract can be found as a sub-heading under Miscellaneous Documents, Bureau of Long Term Care/Model Contracts, within the website. Centraport users can access these documents via the DOH website icon appearing on their desktops.

The updated model contract reflects elimination of the requirement for a reassessment of an ALP resident within 45 days of admission. This amendment was authorized by the Laws of 2000, chapter 569, which amended SSL 461-l(2)(d)(iii). Additionally, the updated version reflects recent changes in Medicaid policy related to payment of the Medical Assistance component for ALP services to mirror Nursing Home policy when a resident is absent from the ALP. The number of days a recipient may be absent from the ALP has been reduced from 18 days to 10 days in a twelve month period. If the recipient's total days of absence exceed 10 days in a twelve month period, prior authorization from the LSSD is required.
