The purpose of this Local Commissioners Memorandum (LCM) is to advise the local districts of $4,907,000 in available State general funds for the continued development of a local point of entry to long-term care services under the program name of NY Connects PY3 that will provide a consumer-centered entry point for information and assistance about long-term care services. This LCM also provides funding allocations for each social services district, explains how the funds may be used, and describes the planning and claiming requirements.
