The purpose of this message is to inform upstate local social services districts (districts) about the revised LDSS-4921(rev 7/09): "Working Families Food Stamp Initiative Screening Sheet" and to advise districts to begin using the 7/09 revised form for all NTA FS applications effective immediately.
