(This Policy Bulletin Replaces PB #08-165-OPE and Obsoletes PB #02-103-OPE)
The purpose of this policy bulletin is to inform Job Center staff of revisions to the Cash Assistance Application Kit Forms (M-90c) and the Cash Assistance Recertification Kit Forms (M-90d). The following changes have been made to forms M-90c and M-90d:
• The Welfare Fraud Pamphlet (W-151M) has been made obsolete and replaced with the Welfare Fraud Brochure (BFI Bureau of Fraud Investigation) (BRC-151M)
• The BRC-151M has been updated to reflect the following:
�� The text has been modified
�� The logo has been replaced with the new NYC logo
�� The form has been redesigned with new graphics
�� A BFI email address, bfifraud@hra.nyc.gov, has been added
�� The names of the program heads
