The purpose of this policy bulletin is to inform Job Center staff of a revision to the Notice of Decision on Assistance to Meet an Immediate Need or Special Allowance (For Applicants Only) Form W-145HH). It is informational for all other staff.
Form W-145HH has been revised to include an additional box on the second page of the form titled “Other action taken on your application.” JOS/Workers will be able to select this box when a type of emergency assistance that is not already listed on the form (i.e. burial allowance) is being granted on an application. When a JOS/Worker selects this box on the form they should also make an entry in the accompanying text box indicating the action being taken on the application.
