The purpose of this policy bulletin is to inform all Job Center staff that the Family Independence Administration (FIA) has developed a new notice entitled Important Notice Regarding Your Eligibility for Cash Assistance and Food Stamps (M-327hh) form and revised the Mail-In Recertification/Eligibility Questionnaire (M-327h), to conform to the State version, form LDSS-4887.
The revisions to form M-327h are as follows:
• The opening statement has changed.
• Public Assistance (PA) is now Cash Assistance (CA) throughout the document.
• In Section 3 the following were added:
�� A new heading, entitled “Source of Income.”
�� A new Box H, entitled “Other Income.”
• In Section 4 under the question “Have there been any changes in the following since you last reported to us?” the following was added:
�� In row D, Child support you pay to someone outside you household;
�� In row E, Medical expenses paid by household member who is disabled or who is 60 years old or older;
�� In row F, Other changes.
