The purpose of this policy bulletin is to inform staff that the Important Information About Child Care Form (LDSS-4647) is now included in the Cash Assistance Recertification Kit.
The Cash Assistance Recertification Kit Form (M-90d) has been updated to reflect the inclusion of the Form LDSS-4647.
