The purpose of this policy bulletin is to remind all Job Center and Food Stamp Office staff that the Notice to All Applicants (EXP-75Q) has ben revised.The reference to the Application/Job Profile (W-680) form has been replaced by reference to the Application for: Temporary Assistance (TA) - Medical Assistance (MA) - Medicare Savings Program (MSP) - Food Stamp Benefits (FSB) - Foster Care (FC) - Child Care Assistance (CC) form (LDSS-2921).
