This policy bulletin is to inform staff that the Notice of Intent to Discontinue Food Stamp Benefits Form (W-129EE) has been revised as follows:
Revisions to Form W-129EE:
• Page 1 – “Notice” was deleted from the date in the participant’s identification information section of the form.
• Page 1 – Date, Case Number, Case Name, Caseload, Family Size, Center and FH&C Telephone Number were reordered in the participant’s identification information section.
• Page 1, Paragraph 3 – “Page two (2) of this notice is a financial fact sheet which” was deleted from the text. The text now reads “The enclosed ‘Your Financial Facts’ shows all of the financial information on our computer file that was used to calculate your revised eligibility, including the amount of the child support incentive payment that has been budgeted.”
• Page 2 – Your Financial Facts, Section E (Food Stamp Net Income), item number 21 was modified. The sentence “The total cannot be more than when combined with the amount on line 10” was deleted from this item.
