This policy bulletin is to inform staff that the Request for Emergency Assistance for an Active Public Assistance Case (W-145N) and Notice of Acceptance/Denial of Request/Application for Emergency Housing Aid (W-145P) have been revised.
Both forms have been designated “LLF” to comply with Local Law 73 requirements and therefore will be available in six foreign languages in addition to English: Arabic, Chinese, Haitian Creole, Korean, Russian and Spanish.
