The purpose of this policy bulletin is to inform staff that the Periodic Report (LDSS-4310) and the Follow-up to the Periodic Report (LDSS- 4310A) have been revised.
Food Stamp (FS) households reporting earned income with seven months or more remaining on their FS certification period will receive the state-issued LDSS-4310 in the fifth month of that certification. This report is used to record any changes to the household’s income.
