The purpose of this bulletin is to introduce a new NYCWAY form, the Reevaluation of Employability Status Appointment (form W-584M). NYCWAY will generate the W-584M when the following action codes are posted: 105L, 105U, 109E, 109F, 109J, 109K, 109V, 110F, 110J, 110Y, 200F, 205C, 205T, 214B, 214M and 214N. A listing of the action codes and their corresponding descriptions appear below for reference. These action codes do not modify the current usage of the codes.
