These FIA forms are now obsolete: • W-560EE Notice of Intent to Change Child Care Benefits • W-560EE (S) Notice of Intent to Change Child Care Benefits • W-560Y Rights and Responsibilities for Transitional Child Care Benefits The ACS forms are: • CS-560EE Notice of Intent to Change Child Care Benefits • CS-560EE (S) Notice of Intent to Change Child Care Benefits (Spanish) • CS-560Y Rights and Responsibilities for Transitional Child Care Benefits • CS-560Y (S) Rights and Responsibilities for Transitional Child Care Benefits (Spanish)
