The purpose of this policy bulletin is to inform staff that the Notice of Recertification Appointment form (W-908T) has been revised to include:
• a paragraph advising participants with children under the age of 13 that the Child Care Provider Application and Voucher Form (W-273B) will be sent in a separate envelope; and • additional lines (for listing individuals) have been added to the verification of school attendance section.
