(This Policy Bulletin Replaces PB #06-141-OPE)
The purpose of this policy bulletin is to inform staff that the following changes to HEAP forms have been made for the 2007—2008 season, which begins November 1, 2007:
• The dates on the HEAP 21, HEAP 23, and HEAP 24 forms have been updated to reflect the current 2007–2008 season.
• The HEAP 6, HEAP 8 and HEAP 18 forms have been revised to reflect the new direct vendor payment rate of $120 to $540.
