The purpose of this policy bulletin is to inform all staff that:
• certain FIA child care forms have been made obsolete through conversion to ACS forms
• certain ACS child care forms have been given the new ACS identifying form numbers
• the Administration for Children’s Services (ACS) has created two new child care forms
• new ACS child care forms have been created to replace certain FIA and ACS child care forms.
