The purpose of this policy bulletin is to inform all Job Center staff that the following utility-related forms have been revised to conform to the new programming requirements:
• Notice to Utility Company of Issuance of Direct Vendor Payment (M-858)
• Notice of Intent to Restrict Home Energy Allowance for Family Assistance Cases (Timely) (M-858c)
• Notice of Intent to Restrict Home Energy Allowance (Timely) (M-858ff) Participant Request for Restriction of Home Energy Allowance (M-858w)
• Notice of Intent to Recoup Utility Grant (Timely) (M-858x)
