This policy bulletin is being revised to inform staff that:
.. Individuals that have an in-person appointment but want a telephone appointment must contact OCSS at the number listed on the OCSS Appointment Notice (OCSS-91), OCSS Appointment Reminder (OCSS-91a), Follow-Up Appointment Notice (OCSS-91b), or Rescheduled Appointment Notice (OCSS-91c) to request a change.
