The purpose of this policy bulletin is to inform all Job Center office staff that the following forms are obsolete:
• Brooklyn Housing Court Palm Card (W-147T)
• Queens Housing Court Palm Card (W-147U)
• Manhattan Housing Court Palm Card (W-147V)
• Bronx Housing Court Palm Card (W-147W)
The aforementioned English-Spanish palm cards were formerly distributed to the public when they had legal issues regarding their housing. The cards are no longer necessary because applicants/participants are now directed to the HRA room where Rental Assistance Unit (RAU) Case Consultants and Associate Job Opportunity Specialists (AJOS) make eviction prevention assessments and referrals. Additionally, the HRA room is also listed in each Court’s directory.
Center Directors must ensure that all previous versions of these forms are removed from circulation and recycled.
