The FIA-1104a informs applicants and participants about the requirements necessary for the Human Resources Administration (HRA) to help with utility payments. It also explains to the individuals what a utility guarantee is, its benefits, its duration and other questions related to how the utility guarantee functions.
The FIA-1104a must be included in the Application Kit and the Recertification Kit effective February 1, 2017. The Cash Assistance Application Kit Forms (M-90c) and the Cash Assistance Recertification Kit Forms (M-90d) have been updated to include the FIA-1104a.
