The purpose of this policy bulletin is to inform Job Center and NCA SNAP Center staff that the New York State Department of Health has mailed Internal Revenue Service (IRS) Form 1095-B Health Coverage and an accompanying informational letter (Attachment A) to all clients who were enrolled in Medicaid and/or Child Health Plus (CHP) in the year 2015.
