This policy directive has been developed to incorporate the following information:
- LOSU has relocated to 109 East 16th Street, 6th Floor, New York City, New York 10003.
- The Division of Accounts Receivable and Billing (DARB) has relocated to 150 Greenwich Street, 34th Floor, New York City, New York 10007.
- The Bureau of Reconciliation and Control (BORAC), Check Replacement Unit, has relocated to 150 Greenwich Street, 34th Floor, New York City, New York 10007. Their new telephone number is (929) 221-6023. Their new fax number is (212) 437-2764.
- The Division of Automated Check Inventory and Tracking (DACIT), has relocated to 150 Greenwich Street, 34th Floor, New York City, New York 10007. Their new telephone numbers are (929) 221-5996/3758/6016. Their new fax number is (212) 437-2764.
- References to the BFI Fraud Referral Log (W-400B) have been removed because it was obsoleted with publication of PD #12-12-OPE.
- The Replacement of Direct Vendor Payments (W-147A) form has been made obsolete.
- The Request for Original or Copy of Checks (M-160j) form has been revised to include the DACIT fax number.
