The purpose of this policy bulletin is to inform all Job Center and Non-Public Assistance Food Stamp Office staff that the following forms are obsolete. This policy bulletin serves as information for all other staff. Of note: Among the forms noted as obselete are two fair hearing forms, the "Fair Hearing Agreement Form (M-186t)" according to the PB, " Fair Hearing no longer uses this form." and "Withdrawal of Fair Hearing Request (M-186dd)" According to the PB, "HRA is no longer permitted to request the withdrawal of a Fair Hearing on an applicant/participant's behalf."
