The purpose of this policy bulletin is to inform Job Center staff that the Cash Assistance Application Kit Forms (M-90c) and the Cash Assistance Recertification Kit Forms (M-90d) have been revised to reflect a change in the form number on the Welfare Fraud (BFI Bureau of Fraud Investigation) (BRC-151) brochure.
