This PB replaces PB #06-138-OPE. It explains that "this policy bulletin is being revised to inform Job Center staff that the Proof of ID and Residency for Your Child Care Provider or "Babysitter" (W-574FF) form, which was made obsolete October 5, 2006, has been reinstated. As before, Job Center staff will issue the W-574FF to applicants/participants who request child care payment for an informal provider at an initial interview or any other contact."
