This PB announces that some forms about ID cards are now obsolete and that the correct forms are "Using Common Benefit Identification Cards (CBIC) for Medical Services form" (W-126E) and Request for Identification Card/Temporary Medicaid Authorization/Update Existing CBIC" form (W-607A). In a typically helpful move, HRA has only attached the obsolete forms to this PB, not the current, correct forms.
