The purpose of this policy bulletin is to inform all Job Center and Non Cash Assistance Food Stamp (NCA FS) Center staff that Form W-145UU has been updated to provide current information on temporary housing facilities, domestic violence shelters, and hotels for individuals/families that are in need of these accommodations. Also included are instructions on how to utilize the W-145UU with POS and FS POS.
Form W-145UU provides the most up-to-date information on daily rates, addresses, telephone numbers, and contact persons, as well as a brief description of temporary housing facilities located within the five boroughs of New York City.
