This policy directive has been revised to announce that the Referral from Cash Assistance for Separate Determination of Medical Assistance Form (M-42f) has been converted to the Cash Assistance Closing/Denial Codes Requiring Separate Medicaid Determinations Desk Guide. This new desk guide reflects the current closing and denial codes that require a Medicaid Separate Determination (MSD).
