Revisions to the Original Policy Bulletin:
This policy bulletin is being revised to:
• include a change to the processing of home visit requests for one-time Emergency Assistance (“one-shot deal“) cases;
• introduce a revision to the Determination of Home Visit Needed/Homebound Status Form (FIA-1028a) to include an HVN/HB status determination approval for over 365 days and a Wellness, Comprehensive Assessment, Rehabilitation and Employment (WeCARE) HVN/HB status determination;
• introduce a revision to the Activities of Daily Living Client Information Form (CAS-102) that includes two new check boxes that applicants/participants should check if they have submitted a supportive housing application (HRA 2010e) within the past twelve months or if they have completed a WeCARE medical assessment within the past twelve months;
• provide instructions for renewing HVN/HB status;
• announce new New York City Work Accountability and You (NYCWAY) Home Visit Needed/Homebound action codes;
• introduce three (3) new HVN/HB forms: Notice of Scheduled Home Visit (FIA-1028h), Documentation Request for Home Visits Needed/Homebound Status (FIA-1028i), and Notification of Expiration of Home Visit Needed/Homebound Status (FIA-1028k);
• introduce the Home Visit Needed (HVN)/Homebound (HB) Status Liaison Checklist of Required Documents for CAS Review (FIA-1028L);
