The purpose of this policy bulletin is to inform staff of:
• Changes to the Child Care Provider Enrollment Supplement
(W-274W)
• The revisions to the English versions of the Enrollment Form for
Provider of Legally-Exempt Family Child Care and Legally-
Exempt In-Home Child Care (OCFS-LDSS-4699) and the
Enrollment Form for Provider of Legally-Exempt Group Child
Care (OCFS-LDSS-4700). (The updated Spanish versions are
not yet available. Staff should continue to use the Spanish
versions dated 1/2005)
• Five additional State forms were created as supplements to the
LDSS-4699 and are necessary to enroll certain provider types:
