(This Policy Bulletin obsoletes CD Memo #83-158)
The purpose of this Policy Bulletin is to announce that the following
forms have been updated to conform with the latest Agency software
and logo:
• M-32f Notice of Requirement to Provide Documentation of Resources at Recertification Interview;
• M-5b Notice of Transfer of Case to Food Stamp Center;
• W-129RR Notice of Food Stamp Recertification Appointment; and
• W-132S Notice of Documentation Required – Change in Household Circumstance.
Additional revision to Form M-32f: The paragraph concerning the maximum resources levels was removed because most Food Stamp households are now categorically eligible.
M-32f Notice of Requirement to Provide Documentation of Resources at Recertification Interview is sent to participants when a computer match shows unreported assets.
M-5b Notice of Transfer of Case to Food Stamp Center informs participants that their case is being transferred to a Food Stamp Center because they are no longer receiving Cash Assistance.
W-129RR Notice of Food Stamp Recertification Appointment advises participants that an in-person recertification appointment has been scheduled for them.
W-132S Notice of Documentation Required – Change in Household Circumstance informs participants that they are required to verify recently reported changes in their household circumstances.
