The purpose of this policy bulletin is to inform Job Center staff that the Notice of Intent to Discontinue Child Care Benefits (W-273YY) form has been revised to reflect the Agency's most current terminology and logo.
In addition, the following fields have been deleted:
• Worker Name, and
• Worker Phone.
