(This Policy Directive Replaces PD #01-14)
This policy directive has been revised as follows:
• The Office of Project Support has taken over the handling of Grants of Assistance for Guide Dogs (GAGD) from the Rental Assistance Unit. All references (including address and phone numbers) to the Rental Assistance Unit have been updated accordingly.
• The Notice of Determination on Application for Guide Dog Food Assistance Program (M-686C) has been revised with the correct address of the Guide Dog Food Program.
• The Application/Recertification Guide Dog Food Program form (LDSS-3087) has been revised with the correct address of the Guide Dog Food Program.
