(This Policy Bulletin Replaces PB #10-27-OPE, and Obsoletes PD #00-89)
The purpose of this policy bulletin is to inform Job Center staff and the Application Kit Unit (AKU) of the Office of Central Processing (OCP) that the Cash Assistance (CA) Application Kit Form (M-90c) has been revised to remove the Access NY Health Care Application (DOH-4220) because it is no longer required in the CA application kit. Job Centers are not required to distribute the DOH-4220.
