The purpose of this policy bulletin is to inform Job Center staff that the Action Taken on Your Request for Emergency Assistance or Additional Allowance (For Participants Only) (W-137B) and Cash Assistance Additional Allowances (W-137C) forms have been revised as follows:

*The Burial Allowance section on form W-137C now states that burial costs may not exceed $1,700 and that the maximum burial benefit amount is $900.
