#09-61-OPE: Revisions To The Begin Managed Program Assignment Information Summary (W-573R) Form

The purpose of this policy bulletin is to inform all Job Center and Employment Service staff of revisions to the BEGIN Managed Program Assignment Information Summary (W-573R) form. Form W-573R is given to participants in the Begin Employment Gain Independence Now (BEGIN) Managed Program by Caseworkers and has been revised as follows:
*The NYC logo has been updated.
*The form has been determined to meet Local Law 73 requirements and will be translated into Arabic, Chinese, Creole Haitian, Korean, and Russian.
*The form has been modified to a fillable format with a drop-down menu to accommodate the following BEGIN Programs and their corresponding schedules:
*Internship Program
*Vocational Work Study Program
*Work Study/Language Work Study Program
