All individuals have the right to either request a Food Stamp (FS) application in-person or request that a FS application be sent to them by mail. FS applications may be submitted by mail, facsimile (fax), in-person, or through an authorized representative. A FS application will be considered as filed if it meets the minimum requirement of containing at least the applicant's name, address (if they have one) and signature (on either page 1 or page 5 of the FS application). The date that a FS application meeting the minimum requirement is received by the Center is considered the FS filing date. FS applications received by mail or fax that do not meet the minimum requirement must be mailed back to the applicant immediately.
