The purpose of this bulletin is to inform all staff that the Office of Temporary and Disability Assistance (OTDA) has revised the Food Stamp Change Report Form (LDSS-3151). The primary reason for this revision is to update the change reporting requirements for non-six-month reporting Food Stamp (FS) households. The requirement of having to report changes within 10 days of a change has been modified to allow these households to report changes within 10 days after the calendar month in which the change occurred.
