How to Request a Fair Hearing in New York State

There are five ways to request a Fair Hearing:

Telephone: Call 1-800-342-3334

Mail: Complete the "I want a fair hearing" section of the notice and mail a copy to:

Office of Administrative Hearings
New York State Office of Temporary and Disability Assistance
P.O. Box 1930
Albany, New York 12201

Fax: Complete the "I want a fair hearing" section of the notice and fax it to: 1-518-473-6750

Online: Go to: http://otda.ny.gov/oahforms/erequestform.aspx and fill in the form

Appear in Person: In New York City, go to:

Office of Temporary and Disability Assistance
Office of Administrative Hearings
14 Boerum Place, 1st Floor
Brooklyn, New York 11201

In Albany, go to:

Problems with Calling for a Fair Hearing

Calling the toll-free telephone number may seem like the easiest method to request a Fair Hearing but there are two serious problems with this approach:

• The Fair Hearing Request Line [1-800-342-3334] is constantly busy.

• Even if your call is answered, there is a long wait to speak to a live person.
