How to Request a Fair Hearing in New York State
There are five ways to request a Fair Hearing:
• Telephone: Call 1-800-342-3334
• Mail: Complete the "I want a fair hearing" section of the notice and mail a copy to:

• Fax: Complete the "I want a fair hearing" section of the notice and fax it to: 1-518-473-6750
• Online: Go to: http://otda.ny.gov/oahforms/erequestform.aspx and fill in the form
• Appear in Person: In New York City, go to:

Problems with Calling for a Fair Hearing
Calling the toll-free telephone number may seem like the easiest method to request a Fair Hearing but there are two serious problems with this approach:
• The Fair Hearing Request Line [1-800-342-3334] is constantly busy.
• Even if your call is answered, there is a long wait to speak to a live person.
